Ranges Community Health is an independent, not-for-profit community health organisation with centres in Lilydale and Belgrave. Our service area is the southwest part of the Shire of Yarra Ranges.
Our mission is to support and promote healthy individuals and communities by providing quality, accessible community health services. This is achieved through the provision of primary health and population health services locally at our centres, at GP clinics and community hubs. Fees vary according to income and an individual's ability to pay with priority access for the disadvantaged and vulnerable members of our community. The wider community has access to services as part of a social and holistic model of health for both individuals and communities.
As we are a not-for-profit organisation a majority of our income is raised through funding, fundraising and donations like yours.
Our staff works closely with other key agencies, community groups and organisations as well as a wonderful pool of volunteers. This collaboration and partnership is invaluable as it enables us to extend our capacity and our role in the provision of health services.
Ranges provides a wide variety of community-based services including:
- Home and Community Care (planned activity groups, allied health)
- Community Health Program (health promotion, child health, allied health, community health nursing, counselling/casework)
- Dental Services (community dental program)
- Chronic disease management (Early Intervention in Chronic Disease program)
Our service provisions include:
- Community Dental Service provides dental health care for all health care and pensioner concession card holders (and their dependants) living in Victoria
- Physiotherapists provide assessment and treatment for musculo-skeletal complaints (back/neck pain, joint pain, poor walking/risk of falls)
- Occupational Therapists can help with home safety assessments and home modifications which may be needed to maximise safety and independence.
- Counsellors can help support you with mental health and wellbeing issues including family violence
- Podiatrists help you maintain good foot health if your feet are at higher risk as result of other conditions (e.g. diabetes)
- Social Support Worker helps connect people who are elderly, frail or have a disability with social activities
- Dietitians help with dietary education and support for children and adults
- Diabetes Educator provides support and education for people at risk or newly diagnosed with diabetes
- No Interest Loan Scheme may be available to help you with the purchase of new essential white goods
- Speech Therapist (for preschoolers) helps with speech sounds, language, social skills and fluency (e.g. stuttering)
- Children's Occupational Therapist (preschoolers and early primary school) helps with coordination and fine motor problems (e.g. pencil grip)
- Children's Psychologist (preschoolers and early primary school) helps with behaviour and development concerns
- Healthy Mothers Healthy Babies is a support service to help vulnerable mothers during pregnancy.
Demand has increased due to ongoing population growth. With our goal of supporting our community to achieve better health outcomes, we need your help.
How will the funds be used?
Every dollar counts and can make a big difference to a member of the community and assist them to lead an everyday life in good health.
Your donation will assist us to increase our service provision and deliver high quality health care to our community, will assist in increasing existing services and developing new initiatives and programs to fulfil the health needs identified in the Shire of Yarra Ranges.
You will be providing support and reassurance to someone not unlike yourself ensuring access to health services delivered locally with a personal individualised approach.
Ranges Community Health will accept tied donations to support particular programs, for resources, new programs or medical equipment if preferred.
