121 Care provides supports for people with disabilities who do not have funded support but are in dire need of services. This is paid for through our Emergency Fund. Our supports to non-funded participants includes personal care, in-home assistance, transportation, as well as community and social supports. 121 Care also helps them prepare funding applications that will pay for their long-term care and supports. People with disabilities are extremely vulnerable to situations such as that presented by COVID-19. Our trained support workers provide in-home support for non-funded participants released from hospital with diagnosed conditions such as motor neurone disease (MND),  multiple sclerosis (MS) and the like; as well as conditions such as amputations, acquired brain injury, and spinal injury to name a few. 

121 Care provides in-home support for people living with a range of disabilities. All our long term clients are supported through NDIS.
